CREATING BALANCE

Generations helps improve the quality of life for patients and their families utilizing appropriate psychiatric crisis interventions for those with symptoms including:

• Depression
• Suicidal attempts or threats
• Loss of appetite, sleeplessness or social isolation
• Psychosis
• Mood disorders
• Anger, aggression or combative behaviors
• Bipolar disorder
• Schizophrenia
• Anxiety-related disorders
• Chemical dependency and substance abuse
• Substantial grief and loss
• Cognitive impairments stemming from mental health origins

Our specialty program includes chemical detoxification (prescription drugs, illicit drugs and alcohol) for individuals with co-occurring psychiatric and substance abuse disorders. Our program also provides ongoing medical treatments for those patients who may require medical assistance. Although we are not a medical surgical hospital, the Generations program is able to effectively treat the underlying physical /medical problems of our patients. Patients who are post-surgical or have debilitating medical conditions such as diabetes, congestive heart disease, wound care, respiratory problems, etc. may all be treated in conjunction with their mental health concerns

ADMISSIONS

When referring someone to the Generations program at Intermountain Hospital, please contact our Assessment and Referral Department at (208) 377-8400. Clinical staff are available 24 /7 to assist with all levels of pre-admission assessment screening to determine if the patient will benefit from our services. If the patient is appropriate for the program, our interdisciplinary treatment team of doctors, nurses, therapists, CNAs and other healthcare professionals will guide the patient towards a path of healing in a secure and therapeutic environment.

ADMISSION CRITERIA

This program is appropriate when:

• Patients are physically and mentally able to participate in and benefit from the program.
• An improvement in the patient’s level of functioning can be anticipated.
• The patient poses an actual or imminent danger to self or others due to a psychiatric disorder. Suicidal patients may be admitted and assessed for their level of suicidality and, based on the assessment, be placed in the most appropriate program and level of care.
• The patient needs continuous, skilled observation, evaluation, and treatment in an inpatient hospital setting.
• The patient’s condition can be expected to deteriorate without an inpatient admission and /or with less intensive treatment.
• The patient has a medical condition or medication sensitivity that complicates treatment of the mental disorder outside the hospital and a more intensive evaluation of the medication regime is required.
• The patient requires an intensive inpatient evaluation of both their current psychiatric conditions and possible contributing medical conditions which adds to the patients’s inability to function.

THE PROGRAM

Based on the initial psychiatric and medical assessment and orders obtained from the admitting psychiatrist, a Master Treatment Plan is initiated. The plan is formally reviewed by the attending psychiatrist and program staff and updated and revised as necessary throughout the patient’s stay. Psychiatric and medical condition problems are identified, short and long term goals are established, and interventions developed by the Treatment Team as well as the patient and family /guardian / legal custodian.

FAMILY MEETINGS

Family meetings and involvement are important parts of the treatment process. The Social Services Department and Program Therapists will establish family meetings during the patient’s stay in order to assist in the overall discharge plan for the patient. Guardians and other responsible individuals will be included as identified.

DISCHARGE PLANNING

Discharge planning is initiated at the time of admission, then addressed throughout the patient’s stay and revised as needed based on patient need and placement requirements. A written Aftercare Plan is developed with  participation of the patient, family members, and the hospital staff. It includes recommendations for further  treatment and /or follow-up appointments, placement considerations, and other post-treatment follow-up activities.

TRANSITIONS

For some patients, this may be a time when life transitions and changes are necessary. For older individuals, this might mean a new living arrangement or additional home care and support. As part of the Aftercare Plan, all housing and placement needs will be evaluated and, with patient and family involvement, a housing /placement plan will be completed.

PROGRAM COST
